By submitting this form and participating in activities, you (“Participant”) agree to the following terms: 1. Assumption of Risk & Release of Liability I understand that participating in fitness activities, challenges, classes, events, and related exercise programs carries inherent risks, including physical injury, sickness, or property loss. I acknowledge that these risks may result from my actions, inactions, or the actions of others. In consideration of being allowed to participate, I, on behalf of myself, my heirs, executors, administrators, successors, and assigns, hereby waive, release, and forever discharge Natural State Fitness, its owners, officers, employees, agents, contractors, volunteers, representatives, affiliates, and any event sponsors (collectively “Released Parties”) from any and all claims, demands, causes of action, liabilities, damages, costs, or expenses arising from or relating to my participation, including but not limited to claims for personal injury, property damage, wrongful death, or negligence. 2. Medical Clearance & Responsibility I confirm that I am physically able to participate in the activities for which I am registering. I understand it’s my responsibility to consult a physician prior to and regarding my participation if I have any health concerns. I agree to follow all safety instructions and policies provided by Natural State Fitness. 3. Media & Content Release I grant Natural State Fitness and its affiliates the irrevocable right and permission to photograph, videotape, record, edit, publish, and otherwise use, reuse, and distribute my name, image, likeness, voice, biographical information, testimonials, videos, photos, or other content I submit (collectively “Media”), in any form, in all media, for promotional, marketing, advertising, educational, or commercial purposes without compensation to me now or in the future. I understand that Media may be used on social media, websites, print materials, advertisements, displays, or other digital or physical formats. Acknowledgment of Understanding I have read this Waiver & Media Release Agreement, fully understand its terms, and sign it freely and voluntarily without any inducement. I acknowledge that by submitting this form I agree to be bound by all terms above.

NSF 1000 LB Club / 650 LB Club Rules • Combine your best bench, squat, and deadlift • Reach 1000 lbs (men) or 650 lbs (women) • All lifts must be completed at NSF
The lifter must maintain contact with the bench throughout the entire lift, including keeping their hips and glutes on the bench at all times. The bar must be lowered under control to the chest and pressed to full lockout. Excessive bouncing, uneven lockout, or loss of contact with the bench may result in the lift not being counted.
Max. file size: 1 GB.
The lifter must use a barbell and perform the squat through a full range of motion. Hip crease must reach at or below the top of the knee at the bottom of the squat, and the lifter must return to a fully upright, locked-out position at the top. Partial squats, unclear depth, or obstructed video angles may result in the lift not being counted.
Max. file size: 1 GB.
The lift must begin from the floor and be completed by standing fully upright with knees and hips locked out at the top. The shoulders must be back and the bar under control at completion. Hitching, bouncing, or failure to fully lock out may result in the lift not being counted. DO NOT DROP AT THE TOP. Failure to control the bar all the way down will result in the lift not being counted

All submitted lifts are subject to review. Natural State Fitness reserves the sole discretion to determine whether a lift meets the required standards and whether it is accepted or disqualified. Any unclear, incomplete, or questionable submissions may be rejected at our discretion.
